You possibly read the book 1001 nights, full of fantastic stories about the old days in the Orient. It sounds like distant history but oil, the black gold, made it possible that these dreams revived from the sandy dessert.

Imagine, only some 40 years ago the people from Abu Dhabi, the Capital of the United Arab Emirates, lived in such shanties covered by palm leafs. I visited the Emirates first time in 1975 and lived there for many years. So I was a witness of that miracle of 1000 and 1 nights.

And in only 40 years the revenues from their oil income catapulted them into the 21^st century – “From Rags to Riches” as a book title describes this miracle.

However, as rich as the former Beduins may be now, they are humans like you and me, and many of them have skin problems such as deep acne scars, pigmentation problems or severe scars from accidents.

Rich as they are they bought all sorts of expensive lasers but operated them with little success. So they called us to give lectures and to teach their doctors and demonstrate how the Dermaroller® can change their life to the better. Also doctors from Saudi Arabia and Iran flew in to Abu Dhabi to attend the meeting to gain practical knowledge and learn all the little, but important tricks to achieve perfect results.

At the end of the four days workshops in many clinics, from Abu Dhabi to Dubai and the Emirate of Sharjah, some doctors admitted that they have worked with cheap copies from China and Korea and never achieved at least reasonable results. Only after they treated their patients with the Original Genuine Dermaroller® they suddenly understood the difference between quality and trash.

In addition it was an honour and a pleasure for us to invite Dr Igor Safonov from Kiev/Ukraine to join Michael and myself on that Oriental Workshop. I personally admire Dr Igor and consider him as one of the best dermatologist I have ever met in my Dermaroller-life.  As far as I am concerned there is no other doctor on earth that knows more about scars and how to treat them successfully – at least in most cases. For scar improvement he does not use the Dermaroller® alone, but mainly, he also developed different techniques that often result in a combination with the medical Dermaroller® and the medical Dermastamp®.

As far as I am concerned he would be the next candidate for the Nobel Price in Medicine. So far scars led a shadowy existence in medicine, but Dr Igor perfectly demonstrated that many people don’t have to live with their disfigurement. I am proud and happy at the same time that I met this gentleman 4 years ago when he asked me to visit him in Kiev to demonstrate the Dermaroller® and Dermastamp®. Since then he is our biggest fan.

What did we learn from this Oriental workshop in one of the richest countries in the world?

Medical Peelings, cosmetic peelings performed by beauticians and also so-called “light” or “fruit” peelings sold as OTC skin care products became very popular in recent years. Nobody actually cares for the term peeling or peel and its meaning. It originates from Latin and means: to remove the outer covering or skin… In comparison to dermabrasion, a mechanically procedure with a fast rotating diamond wheel, any peeling-compound is an acid based formulation. So the easy conclusion is, skin is ablated by acid. And the chemical action of acid is a “silent” (non visible) burning. Acid acts over time and by concentration. Especially high concentrated acids are tricky and difficult to handle. Left on the skin for too long and not properly neutralised by a base will result in scars.

Consumers may think: “My product is so mild is does not harm the skin” – wrong! No doubt after a peeling, the skin looks fresher and younger, especially on women’s skin beyond the thirties. The reason simply is, that the proliferation of new keratinocytes slows down with age, and therefore the skin’s top most layer, the Stratum Corneum, stays longer on the epidermis. The scales are subject to UV radiation and other environmental influences and they loose the natural transparency of keratin scales – they become slightly grayish. If removed by acid the skin gains more transparency. Provided a low acid concentration in a skin friendly cosmetic formulation is used we would not have to concentrate on this subject. However, reality is different. Being amazed by the positive skin response after the initial first trial, people start to exaggerate. Instead of peeling the skin according to instruction, possibly once a month, it is done daily. And each time, a tiny scale layer is removed. On top, we have another problem: Cheap acid products from non-controlled manufacturers and copycats, even the best FDA cannot control. The 5% concentration, written on the product, eventually turns out to be 30% and often more. Also the term “fruit” is misleading. Acid is acid, regardless from where it originates. In the end the consumer pays a high price by cancer in distant years.

The skin has two major enemies for premature aging: UVA & UVB radiation and so called “free radicals” (oxygen triplets). UVA and UVB are the same rays they only differ in wavelength and penetration depth. Our culture connotes that a suntanned Caucasian skin gives us the impression of health and beauty. The rest we understand by just glancing at the beach. Like oil-marinated Herrings to be air-dried they lie side by side – for hours. The grilling session is interrupted only by applying a new oil layer, and a drink for cooling off – so they think. They use cheap Sun Blockers by the Gallons and feel protected. They do not know that these Sun blockers have only chemical UVA/B filters that change their molecular structure by body fluids (sweat) within minutes. The longer they expose to the sun, the more UVA/B is transformed into infrared rays that are felt as heat, and the more they drink and sweat. So the question is, why do they still get sun burned and tanned in the end? The only reasonable answer is: some of these rays must penetrate the epidermis and activate the melanin production in the basal layer. Occasional sun tanning – done with common sense – does not harm our skin too much. It is vital and can recover. But have you ever seen common sense at the beach? – I did not. The skin damage comes over time, often in many years, by changing our genes and their expression. Eventually some of our genes are so badly damaged that their proliferation signals get out of control, they mutate and transform cells into rapid growing cancer cells. In Germany we have over 3.000 deadly cases, in sunny countries (Florida, California, Australia) the death toll must be much higher.

Now you may ask where is the relation of Peeling and sun? Very easy: Our epidermis with the underlying dermis not only is a protection against temperature and moisture loss, the epidermis with its full scale layers also acts as a filter. (See above). If you remove part of these scale layers (about 27 in total) the filtering action is decreased, often down to zero. This again results in intensified gene damage and skin cancer will start much earlier.

What to do? Peeling and sun (also solarium or tanning beds) are the skin‘s worst enemies. Avoid one or the other – best avoid both. A good sun protection is a so-called sunscreen. These are silicone-based formulations with embedded minerals, usually titanium- and zinc oxide. A good (medical grade) silicone screen does not prevent sweating, but protects your skin for many hours. Aged scales can easily be removed mechanically by using the Home Care Dermaroller several times per week – without any additional product. Don Owen, a US gene expert, found out that the tiny micro needles (0.2 mm) of the Dermaroller® stimulate gene expression that results in a faster proliferation of new keratinocytes and therefore thickens the epidermis. Forget peeling!

Everything we do should be done with common sense, even the use of the Dermaroller. But my final question is: were can we buy common sense?

I have many confusions about many collagen products on the market. How does it effect human skin and in skin disease treatment (anti-aging, scar treatment…)? Example:

Have you ever been to Vietnam? If not, join us on a wonderful journey to a country that still seems to be so mystical. Meet a culture of thousands of years. Come into contact with people with a glorious and also a tragic history. See people that have put the past 1000 years of colonisation, occupation and a terrible, unjustified war aside. Encounter a nation full of optimism to create its own future.

With genuine modesty I can say that I have travelled the entire world but Vietnam is something special in any respect. These people welcome you with a warm smile of true innocence that surprises you any moment, 24 hours a day. Hospitality is not the correct term, it is more: Vietnam is all smiles and graces.

Wherever we came in this world for lectures and training we always were welcome and our hosts did outstanding performances, but Vietnam topped them all. To be more precise, I report about our new Dermaroller-Representative Mr. Le Manh Hung and his charming wife Cun. Our 3 days stay was organised like a Swiss precision chronometer. Wednesday all day long we gave lectures to their staff followed by practical hands-on training on several patients with different indications.

We passed on all our skill and knowledge to them – and they really learned fast. The Medical Director, Dr. Quong, had some experience with roller-copies from Korea. Once he had used the Dermaroller for the first time he suddenly knew what is quality. Only with a mild pressure the precision Dermaroller micro needles effortless penetrated the acne scar fibrosis.

For the first time we also could demonstrate the efficacy of our new COOL MASK. Directly after medical needling Lipopetide is applied on the skin followed by the COOL MASK. It is made from pure silk soaked in Hyaluronic Acid in combination with highly effective liposomes. It cools the skin and reduces the erythema (redness) significantly. See the difference on the neck where no Cool Mask was applied.

The training was interrupted by a small lunch break with the famous Vietnamese Noodle Soup, called Bo Bun Hue.

At 7 pm we were invited for the opening ceremony of a Spa sponsored by Dermaroller-Vietnam. A real special event we experienced the first time in our life. It started with a traditional Dragon Dance.

and then the SPA opening was followed by a parade of Vietnam’s most beautiful girls, celebrities and actresses.

Early in the morning a driver waited for us, and accompanied by Mr. Hung and Dr. Quong we were heading East towards the famous Ha Long Bay. It is only 170 km away but taking in consideration the Hanoi traffic and the streets packed with about 7 million motorbikes it was a long journey of about 4 hours. In order to give you a little idea, see the traffic picture I have taken from my hotel room.

But our patience was rewarded a million times – we arrived at the Ha Long Bay. Ha Long means were the dragon came to the water (at least so I remember). With a little bit of imagination over 3000 rocky islands form the scales of a dragon’s tail. An absolute breath taking view of a nature surrounding you, that makes you speechless. Mr. Hung was so generous and chartered a big boat for about 50 people only for the four of us plus the driver and an interpreter for German/Vietnamese. The weather was fine, overcastted but with good visibility. It was just the right light to highlight the contrast between sea, rocks and sky.
Almost silently our boat floated between the rocks, a majestic view I never experienced before. The following pictures only can give you a poor impression of a unique nature surrounding us – you have to see it yourself!

Before we had a delicious lunch on board, we made a short land tour to visit a magnificent illuminated dripstone cave where nature has formed bizarre forms of limestone in the course of millions of years.

It was the same tour back and we all felt each single bone when we arrived Hanoi late in the evening. We somewhat were smashed but extremely happy, and we went to bed early with an unforgettable memory. What’s waiting for us the next day? I asked myself when I felt asleep.

Mr. Hung had organized an official Dermaroller presentation for the Vietnamese Medical Society at Hanoi’s most prestigious Hotel. He had invited 70 doctors, 105 came. The arrival of the Dermaroller Team, Michael and myself, was the talk of the town and the conference hall was far too small.

Mr Hung was an excellent translator and the presentation went very smooth.

After the speech Michael and Dr. Quong treated a patient with acne scars in public, and the audience watched the scene that was recorded by TV.

Thank you Vietnam. Thank you Mrs. And Mr. Hung. We experienced unforgettable moments, and we promise you; we shall come back one day.

We have worked very hard to meet the dramatically increasing demand for Dermarollers from all over the world. So we sat down and designed a robot for a fully automatic microneedle roller assembly – a machine never existed before. It took us over one year to build the first prototype. Now we have 5 units running with a daily capacity 0f 1.500 units a day. 5 more are planned for early spring 2011. Hi Horst – I just wanted to thank you, now I have recovered from my near death experience with Jagermeister, for a fabulous visit.

You know how much effort we have put into the development of Dermaroller and so much of our message is based on the quality of the device, the manufacturing, the quality control, the people and the excellence of German engineering. We have done all of this without ever seeing  the process and knowing that it was a developing project. Basically we backed you and you backed us and this has been shown to be a really good decision. I have to say I saw a great, “cutting edge” high quality process that was very well controlled and managed. The manufacturing process uses advanced German engineering and design that leads the field, we can really use this to advance and enhance the marketing and clinical message and we should really work on this together – “vorsprung durch technik” has worked brilliantly for Audi over the years in the UK & English speaking countries and we can make something similar happen with Genuine Dermaroller, people in white coats, advanced manufacturing processes, complicated shiny machinery it can really help us sell and differentiate ourselves and of course it produces excellent and optimum quality medical devices. You have done a great job building this over a relatively short space of time, well done.

Unfortunately my drinking skills were not up to such high standards, I think I drank the least, was the most drunk and was completely poisoned from the local brew on Thursday. I’ll follow up on all the day to day stuff and the things we discussed this morning Michael.
